ANNOUNCEMENT: UST SENIOR HIGH SCHOOL CONFIRMATION OF ENROLMENT

To all incoming Grade 11 students

Non-submission of the requirements and failure to confirm enrolment on the designated day, automatically forfeits your slot.

 

ATTENTION: ALL QUALIFIED APPLICANTS WITH CONFIRMATION OF ENROLMENT FORM (Incoming College Freshmen and Grade 11)

In the likelihood that your report card (Form 138) is not yet available on your scheduled confirmation of enrollment, you are required to:

(1) download the Conditional Admission Contract,

(2) fill it out, and

(3) secure a certification from your Principal's Office stating your expected date of graduation.

Submit these documents during your confirmation of enrollment.

REMINDERS TO QUALIFIED APPLICANTS WHO ARE PAYING THEIR RESERVATION FEE:

Please do not forget to bring your Reservation Form before proceeding to the bank. You can download the Reservation Form after clicking the reserve button. The bank teller will not accommodate your payment without it. For the Subscriber Name, copy the applicant number indicated on the RESERVATION FORM. Note that there is a zero (0) after 2019 before your applicantnumber. (20190 + 5 digit applicant number).

DIRECTIONS FOR RESERVING SLOTS AND CONFIRMATION OF ENROLMENT:

STEP 1: Log-in using your Applicant Number (found on the test permit) and Surname to view your USTET results.

STEP 2: Click the reserve button.

STEP 3: Click the Submit button to confirm the online Reservation.

STEP 4: Download and print the Reservation Form.

STEP 5: With the accomplished Reservation Form, pay at any Metrobank or BPI branch.

STEP 6: Five (5) working days after paying your reservation fee, download the Confirmation of Enrolment Form, Conditional Admission Contract and the Important Reminders.

STEP 7: Submit all the required documents during the confirmation of enrolment. (Please take note of your schedule.)

STEP 8: Three (3) working days after submitting all the required documents, login at https://pds.ust.edu.ph to accomplish the Personal Data Sheet (PDS) online and print the Acknowledgement Slip.

STEP 9: The Acknowledgement Slip is a requirement for enrolment. With your Acknowledgement Slip, proceed to enrolment. (Please take note of your schedule for enrolment).

Please be informed that the results of the UST Junior High School Entrance Test will be viewed online starting at 5:00 PM, today (March 15, 2019).

Kindly input the applicant number found on the test permit and the student’s surname to access the results.

USTET results for UST Education High School will be posted at the bulletin board of the Education High School, Albertus Magnus Building starting 9:00 AM.

You can only view the results of the program/strand you applied for.  The Office for Admissions (OFAD) will no longer provide you with a hard copy of your results. You may download and print your results for presentation purposes during the Confirmation of Enrolment. To facilitate the understanding of your results, it is important that you read everything first before viewing your results.

I. DEFINITION OF TERMS:

  1. QUALIFIED

    Scores of the applicant met all the required cut-off scores set by the College/Senior High School for the program/strand of choice.  (Note: Each College sets their cut-off scores for the programs they offer.  Cut-off scores vary depending on the weight  each subtest has on the program.  The same is true for the different tracks and strands of the Senior High School.)

  2. FOR INTERVIEW / FOR AUDITION

    The applicant is qualified for interview or audition as an additional requirement for the program or strand of  choice. This does not guarantee admission to the program since the interview or audition is still part of the application procedure.

  3. ON WAITING LIST

    Although the scores of the applicant  met the required cut-off scores set by the College or SHS for the program or strand of  choice, because of quota limitations, he/she shall have to wait for further instructions and can only be considered if slots are still available.

  4. DID NOT QUALIFY

    Scores of the applicant did not meet the required cut-off scores set by the College/Senior High School for the program/strand of choice. An applicant must meet the cut-off scores of ALL sub-test to qualify.

  5. PRIORITY PROGRAM

    This is the program that the applicant is applying to be accepted to. This is where his/her interest lies and wishes to further his/her knowledge and skills in.

  6. ALTERNATIVE PROGRAM

    In the case the applicant does not meet the criteria of his/her priority program, he/she will be considered to this program subject to the criteria set by the College.  Once an applicant qualifies in his/her priority program, his/her alternative program is no longer processed

 

II. GENERAL INSTRUCTIONS/REMINDERS FOR QUALIFIED COLLEGE APPLICANTS ONLY:

  1. Qualified applicants must reserve their slots online and download/print the Reservation Form. 

  2. Pay the non-refundable reservation fee of five thousand pesos (PhP 5,000.00) at any Metrobank or Bank of the Philippine Islands (BPI) branch. To avoid inconveniences, please present the Reservation Form with your applicant number when paying the reservation fee.

  3. You will need your applicant number when you fill out the payment slip at any Metrobank or BPI branch. Present to Metrobank or BPI the Reservation Form with the applicant number for verification purposes.

V. GENERAL INSTRUCTIONS/REMINDERS FOR QUALIFIED SENIOR HIGH SCHOOL APPLICANTS ONLY:

A.  For those enrolling at UST:

  1. Qualified applicants must reserve their slots online and download/print the Reservation Form. 
              
  2. Pay the non-refundable reservation fee of five thousand pesos (PhP 5,000.00) at any Metrobank or BPI branch. To avoid inconveniences, please present the Reservation Form with your applicant number when paying the reservation fee.

B.  For those enrolling at UST Angelicum College:

  1. Submit the USTET printed result and the fully-accomplished AC Medical Clearance / Examination Report (downloadable from UST Angelicum College Website - Admissions) to UST Angelicum College Registrar’s Office located at 112 MJ Cuenco St., Barangay Sto. Domingo, Quezon City.

  2. Fill-out the SHS Information Data form and secure a copy of the Preliminary Processing Form for interview at the Registrar’s Office.

  3. Proceed to the Office of the SHS Principal / SHS Strand Adviser at the 3rd Floor Blessed Jordan of Saxony Bldg. for the interview.

  4. Proceed to the Accounting Office for the non-refundable reservation payment of Five Thousand Pesos (Php 5,000.00). Payment of reservation fee for qualified applicants is from January 29 to February 28, 2019. Failure to reserve a slot and pay the non-refundable reservation fee within the specified schedule will automatically mean forfeiture of your slot.

  5. Present the Official Receipt of reservation payment to the Registrar’s Office for the enrollment schedule and for the guidelines for enrollment.


SPECIFIC INSTRUCTIONS FOR QUALIFIED APPLICANTS WHO HAVE PAID THEIR RESERVATION FEES:


Enrollment Procedure for USTET Qualified SHS

Step 1. Submit the following requirements to the personnel-in-charge at the Registrar’s Office for verification and enrolment system encoding:

  • Certificate of Good Moral Character
  • Original F138/SF 9 (Report Card) from the school last attended
  • 2 pieces 2x2 ID pictures
  • Original and two (2) photocopies of birth certificate
  • Original and two (2) photocopies of baptismal certificate (if Catholic)
  • Recommendation form
  • NCAE result

 Step 2. Secure the Assessment/Sectioning /Payment Notice Form from the Registrar’s Office

 Step 3. Proceed to the Strand Adviser for assessment and for the Track-Strand sectioning.

 Step 4. Proceed to the Accounting Office for payment of the Tuition Fee.

VI. GENERAL INSTRUCTIONS/REMINDERS FOR SENIOR HIGH SCHOOL APPLICANTS ON WAITING LIST STATUS:
